  1. Your nurse, caregiver, or person providing your inpatient or outpatient service. You also may request his/her department director.
  2. The patient relations coordinator, who can be reached at 928 639-6263, or in-house extension 36263, Monday through Friday, 8 a.m.-5 p.m. She can answer questions, provide information, and communicate your concerns to the appropriate department director or administrative team member.
