Going Home

There are four steps to being discharged from the hospital:

  1. Your physician will write a discharge order on your chart.
  2. You will discuss your discharge instructions with your physician and your nurse.
  3. Discharge clearance will be given by the Patient Registration financial counselor after insurance confirmations or special payment arrangements have been completed, including applications for assistance.
  4. When you are ready to go home, a hospital employee will escort you to your vehicle.

Checklist for going home

  1. Have you packed everything you brought with you?
  2. Do you have any valuables deposited in the hospital safe?
  3. Has your physician told you if you need to follow a special diet, take medications or monitor your dressings after you have left the hospital?
  4. If your physician has requested, has our dietitian prepared special nutrition guidelines to take with you?
  5. Have you been given your prescriptions?
  6. Has your physician told you if you need to restrict any activities?
  7. Do you know when you next need to see your physician?

Please try to vacate your room as soon as possible after discharge; we will need time to prepare it for the next patient.
