SUBJECT:     DONOR INFORMATION CONFIDENTIALITY

 

POLICY:        The Foundation for Flagstaff Medical Center will protect the privacy and confidentiality of all donor and prospect personal information according to the following procedures.

 

PROCEDURE:    

 

Information we collect: The Foundation for Flagstaff Medical Center may, from time to time, rent public information (name, address, phone, and/or age) or collect the same information on current and past donors, hospital patients, people who have attended a special event, and those who may request planned giving information. In talking with individuals who want to make a charitable gift to the Foundation, we only collect that information that will enable us to be of assistance in helping the individual develop a gift and/or estate plan that meets his/her needs.

 

Information we do not collect: The Foundation for Flagstaff Medical Center does not collect information regarding health issues, including but not limited to hospital stays, diagnosis, prognosis, etc.  This information is not beneficial to the Foundation and will not be available to the Foundation.

 

Information we disclose: The Foundation for Flagstaff Medical Center releases information only to those individuals who need it to perform their jobs and are authorized to have access to confidential information. Donors may also have access to all the information in their own file, but are not permitted access to other donor information. We will disclose information to government entities or other third parties in response to subpoenas or other legal process as required by law.

 

The Foundation for Flagstaff Medical Center will not exchange or rent its mailing list to any outside organization.

 

Opting out: Donors or prospective donors have the right to, at any time, request removal from our mailing list and fundraising solicitations. To do so, the donor or prospect may contact the Foundation office by phone, mail, email or fax.

 

The protection of personal information is of utmost importance to us.  The Foundation for Flagstaff Medical Center maintains strict practices and procedures to safeguard privacy.
