Pre-admitting to the
Perinatal unit

The staff of Flagstaff Medical Center Women and Infants’ Center welcomes you. We recognize this as an opportunity to provide safe care during your birth experience.

It’s important to pre-register
We request that you telephone the Obstetrics pre-registration desk (928 214-3993) to make an appointment with us to begin the admission process. The ideal time to make this appointment is when you are 32-34 weeks pregnant. This meeting will take about one hour. You may want to bring your partner, however, this is your choice.

We hope that by meeting with the registered nurse, you will become familiar with our services, our setting and be able to tell us what experiences you prefer for the delivery and your entire stay in the Women and Infants’ Center.

Your well-being counts
During the meeting with the nurse, you will have the opportunity to ask questions and talk about your needs and plans. This is the beginning of the development of your own birth plan. We would like your stay at FMC to be a positive experience for you and your family. This planning process assists us to be ready for your arrival.

It will be helpful if you have chosen a doctor who will care for the baby after the birth. You can let the nurse know the doctor’s name during this pre-admission process.

Here’s the procedure
It’s easy to pre-admit. Call the Obstetrics pre-admission desk at 928 214-3993. Ask to make an appointment for your “Profile.” There are several choices of days and times, so schedule the appointment for the date and time most convenient for you. Please leave a message if no one answers and the nurse will call you back.

To pre-register as a patient at FMC please call the Patient Registration department. You may call either 928 214-3569 or
928 773-2069. Please review and complete the Patient Registration form that you will receive and mail it back to FMC using the self-addressed, stamped envelope. This will allow our Patient Registration staff to enter your information into our computers.

When you arrive at FMC, enter the Women and Infants’ Center located just south of the main hospital entrance on the east side of Beaver Street. Proceed to the second floor (Labor and Delivery), pick up the phone mounted on the wall and let us know you are at the hospital for your “Profile.”
